an online environment that allows Associates to enroll in benefits, check the status of medical claims, access plan documents and change addresses, phone numbers or beneficiary information at any point during the year. The EMH Benefits Center is a Call Center staffed with experienced benefit coordinators accessible via a toll-free number, (866)446-6150.

HEALTH
BENEFITS
Health Insurance. We offer four different health insurance plans to our Associates: the Lower Risk Option, the Moderate Risk Option, the Higher Risk Option or High-Deductible Health Plan (Health Savings Account). Coverage begins on the first day of employment. Associates may carry health insurance for themselves, themselves and their spouse, themselves and their child(ren) or family coverage, all at reduced premiums. The deduction per biweekly pay period is according to the following schedule:

Lower Risk Option coverage is 90 percent at an EMH Facility.
Moderate Risk Option coverage is 85 percent at an EMH Facility.
Higher Risk Option coverage is 80 percent at an EMH Facility.
HDHP/HSA Option coverage is 95 percent after deductible.
CHA: Add $30.00 to each of the above plans.
Dental Insurance. All full-time and part-time I Associates are eligible for dental insurance benefits. Coverage begins the first day of the month following date of employment. Two options for coverage are offered. The deduction per biweekly pay period is according to the following schedule:

Orthodontia coverage is available for dependent children under the age of 19, with a $1,000 lifetime maximum benefit per member. Orthodontia coverage is provided within the high option dental plan. It includes the diagnosis and treatment plan, minor treatment for tooth guidance, interceptive orthodontic treatment and comprehensive orthodontic treatment.
Vision Insurance. All full-time and part-time I Associates are eligible for vision insurance benefits. Coverage begins the first day of the month following date of employment. The plan includes a vision exam and lens or contact lenses every 12 months. Frames are covered every 24 months. The High Option plan will provide tints, UV coating, basic progressive lenses and polycarbonate lenses at network providers. Healthy Lifestyle Benefit. This benefit is available on a voluntary basis to recognize Associates who make a dedicated effort to improve their personal health. Eligibility for this benefit is based on a risk factor questionnaire and blood work completed the year before.
INSURANCE
Life Insurance. We pay the entire cost of basic group life insurance coverage for all full-time Associates, which is effective the first day of employment. Coverage is determined by annual base salary and years of service. Associates with up to five years of service receive coverage in the amount of 1.0x annual earnings. Associates with 5+ years of service receive coverage in the amount of 1.5x annual earnings.
Short-Term Illness. We provide this benefit specifically to guarantee that your salary will continue in the event of short-term illness. Short-term illness days are accumulated from the first day of employment at a rate of 6 hours per pay period for full-time Associates, to a maximum of 1040 hours to be used after three months of employment. (Hours are pro-rated for part-time I Associates). Short-term illness benefits are paid after 24 hours of non-work related injury or illness, payable at 60 percent of base earnings.
Short-Term Disability. You may choose supplemental short-term disability coverage, which begins on the first day of the month following date of employment for full-time and part-time I Associates. Benefits provided up to 6 months. Coverage is up to 66 2/3 percent of base salary or supplemental to company-paid short-term illness.
Long-Term Disability. We pay for long-term disability coverage as additional insurance for you after two years of full-time service. This benefit is provided only to full-time Associates. Coverage is 60 percent of base (straight time) earnings beginning after 6 months of disability up to 5 years. Associates may voluntarily purchase coverage to age 65 and during the two-year waiting period.
Additional Insurance. You may pay for additional cancer, ICU, whole life as well as supplemental Associate and dependent life insurance and accident insurance.
FINANCIAL
Section 125 Pre-tax Plan, Flexible Spending Account and Limited Flexible Spending Account.
All health care insurance premiums that you participate in are paid with pre-tax dollars. This reduces your taxable income and the premiums are tax-free. You may also designate money on a pre-tax basis to cover medical expenses not covered under the plan or a separate account for payment of child care expenses. If you participate in the High-Deductible Health Plan and Health Savings Account, you also are eligible for a limited FSA to cover expenses, such as dental and vision insurance.
Matching Retirement Savings Plan. This plan is administered by the Principal
Financial Group. We encourage our Associates to plan for their financial security by contributing to a Retirement Savings Plan. Associates will automatically be enrolled in the plan immediately upon employment at 4%. Ephraim McDowell Health will match this 5% for a total of 9.0% of pay. Associates may increase or decrease contributions anytime. Associates hired prior to January 1, 2006 will be vested after three years of service. Associates hired after December 31, 2005 will be 25% vested after two years of service, 50% vested after three years of service, 75% vested after four years of service and 100% vested after five years of service. EMH will match up to 8 percent of your base earnings, according to the following schedule:
200% match on first 1% deferred
100% match on next 3% deferred
75% match on next 4% deferred
PTO Conversion. During Open Enrollment each year, Associates have the option to redirect one, two, three or four hours of PTO (Paid Time Off) per pay period to offset voluntary benefit premiums, apply contributions to their Flexible Spending Account or the Retirement Savings Plan. This option is designed to offer Associates a choice in applying benefit dollars. This option is only available at the beginning of each plan year.
PAID TIME OFF
Holidays,
Vacation Days & Personal Days. We are committed to providing a flexible system for time off. You will begin accruing paid time off as soon as you receive your first paycheck, and you may take time off with pay as soon as it is accrued. Paid time off consists of vacation days, holidays and personal days, according to the following schedule:

Bereavement Pay. Full-time and part-time I Associates who have completed three months of employment may take off up to 24 hours with base pay in the event of the death of an immediate family member, parents-in-law, grandparents and grandchildren. Time off with pay is provided through the day of the funeral.
ASSOCIATE
SERVICES Education/Development. We are dedicated to hiring the best Associates and keeping them trained and updated in their field. We provide a number of in-service programs and continuing education opportunities to our Associates. We also provide time off with pay for Associates to attend approved conferences, seminars, etc.
Education Assistance. In an effort to retain quality Associates, we offer student loan repayment for hard-to-fill positions paid at 25 percent per year, based on the University of Kentucky’s tuition rate. Tuition reimbursement is also available to Associates who are ineligible for student loans. Repayment is 6 months of full-time service for each semester paid. These benefits are available after 6 months of full-time employment and one year of part-time or flex employment for hard-to-fill positions. Eligibility requirements may be waived for Associates who sign a work repayment agreement.
Child Care. All Associates enjoy a 30 percent discount from the standard service rate at A Children’s Place. Child care, preschool, before/after school and summer programs are available.
McDowell Wellness Center. All full-time and part-time I Associates are eligible for discounted rates at McDowell Wellness Center, which offers a variety of fitness programs and an indoor pool for your health and wellness.
MedSource Dicount. All Associates are eligible for a 15 percent discount on items purchased at MedSource, a supplier of home medical equipment. The discount can be applied to items such as uniforms, shoes, etc.
Medication Therapy Management (MTM). Associates may schedule a consultation with one of our pharmacists to discuss prescriptions taken on a regular basis. Participants of an Ephraim McDowell Health health plan would then be eligible for prescriptions purchased through the Outpatient Pharmacy at a 50 percent discount of the plan co-pay amount.
Smoking Cessation Program. This program is held at McDowell Wellness Center. Upon successful completion of the program, we will refund the fee you paid, minus the supplies.
Associate Health Service. After you accept employment with us, we will schedule you for an appointment with our Associate Health Nurse, who will provide your TB screen, review your vaccinations and complete a drug screen. Free vaccinations for hepatitis, tetanus, flu, chicken pox, rubella and rubeola are available to clinical and other Associates who are at risk. Free annual lab testing (basic metabolic panel, lipid profile and complete blood count tests) are also available for Associates.
Associate Assistance Program. We will provide you and your immediate family members with four visits annually (per family) for counseling at no charge.
Discount Services. We routinely offer our Associates discounts on entertainment, etc.
Direct Deposit. Your paycheck will be automatically deposited in banks of your choice. Free checking is available at some local banks. |
